Dr. Kutsy performs USCIS immigration medical examinations and completes Form I-693 as a USCIS-designated civil surgeon.
USCIS examination guideDOT/CDL medical examinations
Call before scheduling so the staff can confirm current DOT/CDL requirements. Bring photo identification and any employer, DMV, or DOT paperwork you were given.
Sports and school physicals
Bring the school, camp, or team form, immunization record if requested, and any instructions from the organization.
Referrals and additional care
Some concerns require a specialist, imaging, laboratory work, or care outside this office. The practice can refer patients to other medical professionals when needed.
